MODEL RELEASE

and ________________________________ hereinafter referred to as the photographer.
In consideration of my engagement as a photographic figure and workshop model and for good and valuable consideration herein acknowledged as received, I hereby grant the photographer, his heirs, legal representatives and assigns, those for whom the photographer is acting, and those acting with his authority and permission, the absolute right and permission to copyright, in his name or otherwise, and use, re-use, publish, re-publish photographs of me or in which I may be included, in whole or in part, or composite or distorted in character or form, without restrictions as to alterations, in conjunction with my own name, or any assumed name, or reproduction thereof in monochrome, color or otherwise made through any and all media now or hereafter known, for illustration, promotion, art,
or any other purpose whatsoever. I also consent to the use of any text used in conjunction therewith. I hereby waive any right that I may have to inspect or approve the finished product, or products and the
text copy or other matter which may be used in conjunction therewith, or the use to which it may be applied. I agree that this release shall be valid for: editorial, advertising, commercial, artistic, and all
other categories of photography. I hereby agree to release, discharge and agree to save harmless the photographer, his or her heirs, legal representatives and assigns, and all persons acting under his or
her permission or authority, or those for whom he or she is acting, from any liability by virtue of blurring, distortion, alteration, optical illusion, electronic manipulation, or use in composite form,
whether intentional or otherwise, that may occur or be produced in the taking of said photographs or in any subsequent processing thereof, including without limitation any claims for libel or invasion of privacy. I hereby warrant that I am of legal age and have the right to contract in my own name, that I have read the above authorization, release and agreement, prior to its execution and I am fully familiar with the contents thereof. This release shall be binding upon me, my heirs, my legal representatives, and assigns.
The model does hereby warrant that she, or he, has received the following in full and complete compensation for all services rendered under this release:
